OSM makes a sincere effort to adhere to the appointment schedule. Emergencies or hospital surgery may occasionally create a delay, and the team appreciates your patience.

Fees and payments

Payment arrangements for office services are requested at the time services are rendered unless prior arrangements have been made. If OSM participates with your insurance company, the office will bill the insurer and assist with claims. Payment remains the patient's responsibility.

How do I request an appointment?

What should I bring to my appointment?

Bring requested forms, photo identification and insurance information. Also bring any records, imaging or imaging reports OSM asked you to provide.

How do I confirm which office to visit?

Check the location shown in your appointment information, then review the current addresses, hours, parking information and directions on OSM’s Locations & Contact page.

Do I need a referral or prior authorization?

Requirements vary by insurance plan and by the care being requested. Check with your insurance plan before the visit, or call OSM at 503-224-8399 if you need information for a referral or authorization.

What should I do with outside records or imaging?

Call OSM before your appointment for instructions about records or imaging from another office. Bring any records, images or imaging reports the OSM team has asked you to provide.

How do I know which form to complete?
